Welcome to Tier 1 Pharmacy Consulting. We are passionate about collaborating and developing effective strategies to improve health plan outcomes.
Whether the task is as simple as writing a policy to meet a new regulatory requirement or as complex as reengineering a website to improve the member experience, it takes energy and teamwork to realize our common goals. That is the basis for everything we do – and my personal commitment to our clients.
Tier 1 creates partnerships focusing on better health plan performance and added value to each and every member. As a pharmacist and the company’s founder and CEO, I have learned there are so many ways to have an impact in healthcare, including direct involvement in patient care while working as a clinical pharmacist and by working at a PBM where solutions and best practices provide the highest quality outcomes and experiences for thousands of members.
I’ve been a pharmacist my entire professional life. I also spent ten years working for an independent mid-market PBM that gave me the experience, insight, perspective and knowledge needed to help health plan administrators navigate a complex, ever-changing and heavily regulated industry.
I understand how an effective PBM helps cut healthcare costs by ensuring regulatory compliance, drug utilization and patient adherence. I get how multiple health plans use different clinical approaches or policies and procedures to seamlessly and effectively implement specific requirements.
That knowledge led to me to launch Tier 1 and offer health plans a new perspective on how to best approach – and manage – their pharmacy benefit. We are experts in making effective plans stronger as well as improving areas that may be problematic due to time constraints or lack of resources.

Brent A. Hiley, PharmD, MBA
Dr. Brent Hiley has spent the last 20 years working in healthcare. Prior to starting Tier 1 Pharmacy Consulting, Brent spent over ten years at an independent, middle market PBM. There, he held a variety of positions ranging from clinical authorizations coordinator to director of account and product management. His time at the PBM taught him every aspect of the industry, including development of prior authorization protocols, oversight of PA reviews, day-to-day account management, new client implementation, regulatory compliance, policy and procedure writing, clinical program development, medication therapy management consultation, P&T committee reviews, RFP writing, contract negotiation, and program audit coordination. The experience, knowledge and expertise he gained at the PBM inspired him to launch Tier 1 to help health plans everywhere reach their goals and outcomes — and improve patient care.
Brent has also worked at a large retail chain pharmacy and as a clinical pharmacist in critical care and emergency medicine at the University of Nebraska Medical Center (UNMC) in Omaha, Nebraska. During that time, Brent helped lead the hospital’s expansion of full clinical pharmacy services into what remains one of the largest emergency departments in the Midwest.
Brent received his Doctor of Pharmacy from the University of Iowa and completed his residency in pharmacy practice at UNMC. He earned board certification in pharmacotherapy (BCPS) and has an MBA in finance from Bellevue University.
